In recent years, our hospital attached importance to the connotation construction. The medical service department organized clinical departments to carry out a seminar on the difficult cases and attending physicians’ classical rounds of wards on a monthly basis and each seminar greatly improved the physicians' clinical ability.
On the afternoon of December 23, 2016, the Second Internal Medicine Department provided difficult cases and organized the seminar, where Yang Xiaohui chief physician and Director of Director of Endocrinology Department of Dongfang Hospital of BUCM was invited as an expert. Chen Minhua attending physician made a report on attending physicians’ rounds of wards for diabetic ketoacidosis and brittle diabetes and the department head made a report on the disease diagnosis and treatment difficulties and the relevant experience. The seminar was attended by all the staff of the Medical Service Department, all clinical department heads, head nurses and clinicians.
Yang Xiaohui first affirmed our accurate diagnosis of patients and timely and effective treatment, and made the classic comments on the case, pointing out that the focus of correcting diabetic ketoacidosis is fluid infusion, potassium supplementation and blood sugar reduction, with cautious alkali addition, while closely observing the patient's cardiac function and urine volume. On the premise of the clinical guidelines, doctors must treat patients differently based on their specific conditions. For critically ill patients, the hypoglycemic treatment may use short-acting insulin analogues as far as possible to ensure slow hypoglycemic process, thus avoiding hypoglycemia. He also noted that once hypoglycemia occurs to a patient, it may destroy all the past efforts in treating diabetes, and also indicated that the close cooperation between doctors and nurses is very critical to the treatment.
The patient's diagnosis and treatment process allowed the participants to feel the rapid changes in the condition of critically ill patients, which enhanced the clinical experience of the clinical doctors in dealing with critically ill patients. In the future, we will continue to discuss and summarize the difficult cases of the department, improve clinical skills and contribute more to the health of patients.
